From what we've seen, the basic kindergarten application form is fairly short and straightforward — usually a single page (both sides). Based on our experience, talking to our friends, and flipping through some books, here's our understanding of what is most common on these forms: - Your Child
- Gender
- Birthdate
- Birthplace
- Student of Color (listed as yes/no checkbox)
- Ethnicity (some forms described this as optional)
- Current School (or Schools Attended)
- Names of Teachers Completing Recommendations
- To which other schools are you applying?
- Attach a photo (of those schools requesting this, some requested one of the child and some requested one of the family)
- Parents/Family
- Occupation (of each parent)
- Employer (of each parent)
- High school/college(s) attended (of each parent)
- Names and Ages of Siblings (and Current Schools)
- Identification of Primary Household or Custodial Parent (where applicable)
A few notes:The question, "To which other schools are you applying?," appeared on only one of the three applications that we completed, so we’re not sure how common that one is. We included the question on parents’ educational background in the lists above; and this seemed to be a common question based on other sources we've consulted. But of the three private schools to which we applied, only one asked for that information. The other two did not expressly ask for that information.
